Opiate Case Management
Pinewood's Opiate Case Management Program strives to meet individual needs of people with opioid dependence through a case management model of coordinated and integrated services. The service provides intensive individual support, assessment, case management, linking advocacy, and flexibility to meet client needs.
The Opiate Case Management Program provides support to people who are dependent on opioids (pain killers such as codeine, Percocet, oxycodone, hydromorphone, and fentanyl). This program is suitable for anyone new to treatment or for people who are already enrolled in opioid replacement treatment such as methadone and suboxone. Working with a case manager, the client develops a personalized treatment plan. This is a low barrier, strengths-based service with a goal of rapid access for clients.
Clients referred to this program must be willing to attend services on a weekly/biweekly basis, establish, and work towards goals regarding opioid use and is limited to a maximum of 24 sessions. The following are the necessary criteria that are required for consideration to the Opiate Case Management Program.
Client must meet one of these criteria to be eligible for the program
- Use of opioids in the past six months;
- Contemplating or has recently started opioid replacement therapy (suboxone or methadone).
Services are covered by OHIP* and are confidential. *Please Note: The Back on Track Program is not covered by OHIP.
