Patient Declaration of Values
Lakeridge Health is committed to providing our patients, clients, residents, and their family members/essential partners-in-care with a safe, high-quality care experience and to continuously improve in order to do so.
Under the Government of Ontario’s Excellent Care for All Act, all hospitals in the province are required to have a Patient Declaration of Values in place that has been developed in consultation with the public and is accessible to the public. Click here to read the Excellence Care for All Act.
What is a Patient Declaration of Values?
A Patient Declaration of Values is a list of values, developed in partnership between the organization and patients/residents/clients and families/essential partners-in-care. The values reflect what is important to the community and team members when receiving and providing care.
Lakeridge Health’s Guiding Principles of Partnership
In 2019, Lakeridge Health formed a working group of Patient and Family Advisors and team members from across the organization to co-design the Guiding Principles of Partnership. The Guiding Principles of Partnership serves as Lakeridge Health’s Patient Declaration of Values and aligns with the organization’s commitment to Person- and Family-Centred Care.
Person- and Family-Centred Care is an approach to the planning, delivery, and evaluation of health care that is grounded in mutually beneficial partnerships among patients, clients, residents, families/essential partners-in-care, and the health-care team.
As a result of this work, the Patient Declaration of Values has been renamed Lakeridge Health’s Guiding Principles of Partnership: A Commitment to Person and Family-Centred Care.
The Guiding Principles of Partnership closely aligns with the four core principles of Patient and Family-Centred Care as outlined by the Institute for Patient and Family-Centred Care:

- Information Sharing
- Collaboration
- Participation
