Your Privacy

Lakeridge Health is committed to protecting patient privacy and places the highest value on maintaining the confidentiality and security of personal health information (PHI).

We’re proud to partner with six other hospital organizations in the Central East region to provide patients with a single, unified digital health record through the Epic Clinical Information System.

  • We take steps to ensure that everyone who performs services for us protects your privacy and only uses your personal health information for the purposes you have consented to.

Requesting Access to Information under FIPPA

You may request access to information at Lakeridge Health under the Freedom of Information and Protection of Privacy Act (FIPPA). FIPPA provides members of the public with a general right of access to information within our hospital’s custody or control as of January 1, 2007 (with limited exceptions and exclusions).

Anyone can make a formal request under FIPPA by mailing or delivering a written request to Lakeridge Health’s Privacy Office at:

Request to Access General Records Form

For your request to be considered complete it must meet the following criteria:

  • Be made in writing and directed to Lakeridge Health’s Privacy Office.
  • Include a $5.00 application fee in the form of a cheque or money order made out to Lakeridge Health.
  • Provide enough detail for Lakeridge Health staff to locate the information you are requesting with a reasonable search. If we require additional information, you will be contacted.

In addition to the $5 application fee, the fees listed below may also apply to your request (as permitted by FIPPA). For each access request received, Lakeridge Health will provide you with a fee estimate. If the fees associated with your request are estimated to exceed $100, we will require a 50 per cent deposit prior to proceeding. We will always advise you of any fees up-front, before processing your request.
